Hall, Room, Meal Plan Changes and Room Damage Form
This form is used to change your meal plan from the time you are assigned to a residence hall and room before the semester begins and up until the 5th Class Day. You can always increase your meal plan at any time during the semester (additional charges will be applied) but downgrades of your meal plan must be submitted by the 5th class day.
The Room Damage Form will be completed as you move into your room. Please fill out any damages you find in your room and common living space and submit the form. Please be as thorough and specific as possible. Any maintenance issues you find can be reported at /housing/resources/ReportingMaintenance.html or to the hall front desk. You will have 10 days from the check-in date to complete the form and submit it. Failure to turn in the form by the deadline could result in charges to the student for any damages found upon check-out.
